MobilityWoRx PT, LLC ("we," "us," or "our") operates the website at https://mobilityworxpt.com (the "Site") and provides pelvic floor physical therapy, orthopedic physical therapy, personal training, and dry needling services in Red Bank, New Jersey and the surrounding Monmouth County area.

Important: this policy covers our website

The website itself is intentionally designed not to collect protected health information (PHI). For information about how we handle PHI as a healthcare provider (including patient records, treatment notes, and information collected through our scheduling and intake systems), see our HIPAA Notice of Privacy Practices.

What we collect on this site

Information you provide

When you submit our contact or appointment-request form, we collect the information you choose to provide. Typically:

• Name
• Email address
• Phone number
• The service you're interested in
• The free-text message you write

The form explicitly asks you not to share medical details, diagnoses, or other protected health information through the site. If we need that information from you, we collect it through a HIPAA-compliant channel (a phone call, in person, or via our secure scheduling system).

Information collected automatically

• Analytics. We use privacy-friendly site analytics to understand which pages get traffic and how the site performs. These analytics do not use cookies or build profiles tied to your identity.
• Server logs. Our hosting provider (Vercel, Inc.) automatically logs information necessary to operate the site, including IP address, browser type, and pages requested.

Third-party services we use

• Vercel. Hosting and edge delivery.
• Web3Forms. Receives and forwards contact form submissions to our email.
• Google. Google Search Console (indexing), Google Business Profile (listings), and (where used) Google Analytics 4. If GA4 cookies are deployed, they will be disclosed and you'll have the option to opt out.

Each of these providers operates under its own privacy policy. We do not sell or rent your information to third parties.

How we use the information

• To respond to your inquiry or appointment request
• To improve site performance and content based on usage trends
• To meet legal, regulatory, or operational obligations

Cookies

The Site itself does not set tracking cookies. If we deploy Google Analytics 4, that service may set cookies as described in its own privacy documentation; you can opt out of GA4 through your browser settings or Google's opt-out tools.

Children's privacy

The Site is not directed to children under 13 and we do not knowingly collect personal information from children. Parents or guardians arranging care for a minor patient should contact us directly rather than through the Site form.

Your choices and rights

You can request to access, correct, or delete the personal information we've collected about you through the Site by contacting us at drmel@mobilityworxpt.com. We'll respond within a reasonable timeframe and in accordance with applicable law.

Data security

We use commercially reasonable measures to protect the information you submit. The Site is served exclusively over HTTPS. No system is 100% secure; if we ever become aware of unauthorized access affecting your information, we'll notify you and the appropriate authorities as required by law.

We may update this Privacy Policy from time to time. The "last updated" date at the top of this page reflects the most recent version. Material changes will be highlighted on this page when they take effect.
